The Mental Health Association of Nassau County, Inc. ("MHANC," "we," "us," or "our") respects your privacy and is committed to protecting the personal information you provide when using SAFE Workplace .

This Privacy Policy explains how information is collected, used, maintained, and disclosed when you access or use the SAFE Workplace mobile application, web-based application, related services, and associated resources.

SAFE Workplace is a program of the Mental Health Association of Nassau County, Inc., located at 16 Main Street, Hempstead, New York 11550.

By using SAFE Workplace, you acknowledge the practices described in this Privacy Policy.

1. About SAFE Workplace

SAFE Workplace is a non-clinical workplace mental health education and support platform designed to help individuals and organizations create healthier, more supportive, and less stigmatizing workplace cultures.

2. Information We May Collect

The information collected depends on how you use SAFE Workplace.

Account and Contact Information

When you create or use an account, register for training, or communicate with us, we may collect information such as: first and last name; email address; telephone number, if provided; organization or employer; job title or role, if provided; username or account credentials; training registration information; and other information you voluntarily provide.

Training and Participation Information

SAFE Workplace may collect information related to participation in SAFE programs, including: training registrations; training attendance or completion; certificates earned; trainer or participant status; organization affiliation; participation in Train-the-Trainer activities; and use of SAFE resources.

Surveys and Assessments

SAFE Workplace may collect responses to surveys, evaluations, or organizational assessments, including: pre-training surveys; post-training surveys; follow-up surveys; training feedback; workplace mental health attitudes and perceptions; behavioral-intention measures; SAFE Workplace Readiness Assessment responses; and organizational feedback.

Some survey or assessment responses may relate to mental health attitudes, experiences, workplace culture, or help-seeking.

Users should not provide medical records, clinical documentation, diagnostic records, or other information that is not requested by SAFE Workplace.

Whenever possible, project evaluation and reporting will use aggregate or de-identified information rather than personally identifying individual responses.

3. Device, Usage, and Technical Information

When you access SAFE Workplace, certain technical information may be collected automatically by the application, hosting platform, or service providers supporting the application. This information may include: device type; operating system; browser type; Internet Protocol (IP) address; device or other identifiers; application version; login or authentication information; pages, screens, or features accessed; date and time of access; application performance information; diagnostic information; and other technical or usage information necessary to operate, secure, maintain, and improve the application.

We may use cookies, session technologies, or similar technologies when necessary to authenticate users, maintain application functionality, remember preferences, protect accounts, or understand general application usage.

4. How We Use Information

MHANC may use information collected through SAFE Workplace to: create and manage user accounts; register users for SAFE training; communicate about training, certificates, resources, or program activities; provide SAFE Workplace resources and educational content; administer surveys and assessments; generate or manage training certificates; provide organizational technical assistance; administer the SAFE Workplace Readiness Assessment; support Train-the-Trainer activities; maintain the SAFE Anti-Stigma Resource Library; monitor application performance and functionality; protect the security and integrity of the application; troubleshoot technical problems; evaluate SAFE Workplace programming; understand whether training contributes to changes in knowledge, attitudes, behaviors, or workplace culture; improve SAFE Workplace programs and services; comply with legal, regulatory, contractual, or reporting obligations; and respond to questions or requests from users.

5. Mental Health and Survey Information

SAFE Workplace is a mental health education and stigma-reduction platform. Certain surveys, assessments, or voluntary user interactions may involve information concerning mental health attitudes, workplace experiences, or help-seeking.

MHANC will use this information for purposes related to SAFE Workplace programming, evaluation, education, organizational improvement, and program administration.

MHANC does not use SAFE Workplace survey information to make employment decisions about individual users.

Where evaluation findings are reported to participating organizations, funders, partners, or other stakeholders, MHANC intends to use aggregate or de-identified information whenever reasonably possible.

6. How Information May Be Shared

MHANC does not sell your personal information . Information may be shared only as reasonably necessary for operation of SAFE Workplace or as permitted or required by law.

Service Providers

We may use third-party service providers to support functions such as: application hosting; account authentication; cloud storage; email or notification delivery; application maintenance; security; analytics; technical support; and other technology necessary to operate SAFE Workplace. These providers may process information only as necessary to provide services on behalf of MHANC and subject to applicable contractual, privacy, and security requirements.

Participating Organizations

If SAFE Workplace is being provided through an employer, school, community organization, or other participating organization, MHANC may provide that organization with information regarding overall program participation, training completion, organizational assessments, or aggregate evaluation results. Individual survey responses will not ordinarily be shared with employers or participating organizations in personally identifiable form unless the user has been informed and has provided appropriate authorization or disclosure is otherwise permitted or required by law.

Legal Requirements

MHANC may disclose information when reasonably necessary to: comply with applicable law, regulation, court order, subpoena, or lawful government request; protect the rights, safety, or property of MHANC, SAFE Workplace users, or others; investigate suspected fraud, misuse, security incidents, or unlawful activity; or protect against legal liability.

7. Data Retention

MHANC retains personal information only for as long as reasonably necessary to: operate SAFE Workplace; maintain training and certification records; fulfill the purposes described in this Privacy Policy; meet program, grant, contractual, accounting, or reporting obligations; comply with applicable law; and resolve disputes or enforce agreements.

Information that is no longer required will be deleted, de-identified, or securely retained until deletion is reasonably possible.

Aggregate or de-identified information that no longer identifies an individual may be retained for program evaluation, research, quality improvement, historical, or statistical purposes.

8. Account and Data Deletion

Users may request deletion of their SAFE Workplace account and associated personal information. Account deletion requests may be submitted through our public account-deletion page: https://mhanc-safe.base44.app/delete-account

Users may also contact MHANC using the contact information listed at the end of this Privacy Policy.

Following a valid deletion request, MHANC will delete or de-identify personal information associated with the account unless retention is necessary for legitimate legal, regulatory, accounting, contractual, fraud-prevention, security, certification, or other permitted purposes.

Certain records may need to be retained for a limited period after account deletion when required by law or legitimate organizational obligations.

9. Security

MHANC takes reasonable administrative, technical, and organizational measures designed to protect information against unauthorized access, loss, misuse, alteration, or disclosure. These measures may include authentication controls, access restrictions, secure hosting, data-security practices, and other safeguards appropriate to the nature of the information.

However, no Internet transmission, application, or electronic storage system can be guaranteed to be completely secure. Users are responsible for maintaining the confidentiality of their login credentials and should notify MHANC if they believe their account has been compromised.

10. Third-Party Links and Resources

SAFE Workplace may contain links to websites, services, resources, or organizations operated by third parties. MHANC does not control the privacy practices of third-party websites or services. When you leave SAFE Workplace and access another website or service, that provider's privacy policy and terms will apply. Users are encouraged to review the privacy practices of third-party services before providing personal information.

11. Children and Age Restrictions

SAFE Workplace is intended for adults and is not directed to children under the age of 13. MHANC does not knowingly collect personal information from children under 13 through SAFE Workplace. If MHANC becomes aware that personal information from a child under 13 has been collected without appropriate authorization, reasonable steps will be taken to delete the information.

12. Your Privacy Choices and Rights

Depending on applicable law and your location, you may have the right to request: access to personal information maintained about you; correction of inaccurate personal information; deletion of personal information; information about how personal information is used or disclosed; or other rights provided under applicable privacy laws. Requests may be submitted using the contact information below. MHANC will respond to valid requests in accordance with applicable law.

13. California Privacy Rights

California residents may have additional rights under applicable California privacy laws, including rights relating to access, correction, deletion, and information regarding the collection and disclosure of personal information. MHANC does not sell personal information. California residents may contact MHANC using the information below to submit an applicable privacy request.

14. Communications

If you provide your email address or telephone number, MHANC may use it to communicate with you regarding SAFE Workplace activities, including: account administration; training registration; training reminders; certificates; program resources; technical support; important service updates; and other communications related to your participation. Where applicable, users may opt out of non-essential communications. If SMS/text messaging is offered and you have opted in, you may opt out by following the instructions provided in the message or by contacting MHANC.

15. Changes to This Privacy Policy

MHANC may update this Privacy Policy periodically to reflect changes in SAFE Workplace, legal requirements, technology, or organizational practices. When changes are made, the updated Privacy Policy will be posted on this page and the Effective Date will be revised. Users are encouraged to review this Privacy Policy periodically.
